Job overview
Cognivate is an independent interdisciplinary team which provides specialist neurorehabilitation to adults with acquired brain injury [ABI]. For each client we bring together the relevant combination of neuropsychology [NP], occupational therapy [OT], physiotherapy [PT], and speech and language therapy [SLT] to enhance their independence, participation, and quality of life by working with them in their homes and community settings, places of work/education, and online.
You will be joining a core team of lead therapists in each discipline who work closely together to ensure excellence in all aspects of our service. As our Lead PT you will be centrally involved in strategic developments, and will provide clinical supervision and guidance to other therapists as appropriate. Over the last four years, we have expanded our geographical reach by involving experienced associate therapists to work with us, and in addition to managing your caseload, you will oversee their involvement and the quality of our overall PT provision.
This is an exciting role for a skilled clinician who thrives in working as part of an interdisciplinary team and is keen to apply and develop leadership skills. You will benefit from individually tailored CPD and external professional supervision, so this will be an excellent opportunity to extend your existing repertoire of expertise – for example by developing new specialisms or by stepping into the independent sector. Overview of the Service and Clinical Framework
Cognivate is a specialist interdisciplinary team [IDT] which provides community-based rehabilitation to adults with ABI. Established in January 2021 by Professor Jane Powell (our CEO and neuropsychologist) and two co-founders, we are a private independent provider.
Our Clinical Director, Dr Anita Rose, is a highly experienced clinical neuropsychologist. She oversees and provides clinical line management to all the other Lead Therapists (physiotherapy, occupational therapy, speech and language therapy). We have a growing network of associate therapists in each of the four disciplines; these are independent practitioners who we commission on a client-by-client basis to work alongside, and guided by, our core team members. They are fully embedded within our systems and processes, and our Lead Therapists ensure that they have the clinical supervision and guidance they need in order to ensure the overall excellence of our provision.
Our Chief Operating Officer, Lydia Storr-Meddings, runs all of our administrative and logistical operations with support from a full-time administrative assistant. These include liaising with all therapists, and especially Lead Therapists, to pull together appropriate interdisciplinary teams for each client; providing the scaffolding/administrative organisation for reports, quotes, meetings, etc; keeping the team informed of business needs, priorities, and data; co-leading and supporting the team with strategic developments; and facilitating a strong team culture, with regular social as well as professional events. She line manages all members of the team from a business perspective, working in close liaison with Anita to ensure that clinical and business management are aligned.
Jane, Anita and Lydia constitute Cognivate’s Executive Team, and report into our Board of Directors [BoD] (which has four members, of whom Jane is one). The BoD is responsible for ensuring Cognivate’s probity, overseeing our financial operations, and advising on/approving major financial and strategic decisions.
Clinical Framework
Although some of our clients are self-funding the majority are referred and funded through medicolegal routes. In every case we work in close liaison with the client, key family members, and any other professionals who are involved in their overall care and rehabilitation; this often includes case managers, solicitors, and other independent or NHS clinicians.
We provide goal-focused interventions and support tailored to the individual needs and resources of people whose ABI has given rise to problems across a wide range of severity and complexity. Some are highly dependent on others for care in many areas of their life; others may be managing their home lives entirely independently but are encountering some difficulties with resuming important leisure, education, or work activities.
We evaluate our practice at both client and service level, and we undertake and/or collaborate in research designed to ensure excellence in our interventions and to contribute to advancing rehabilitation practice nationally and internationally.
At present key elements of our provision include:
- Rehabilitation in the client’s home and local community: We take a highly integrated problem-solving approach in which our therapists work with the client and their families/carers to enhance independence in activities of daily living, social participation, and overall quality of life.
- Vocational rehabilitation: Led by our OTs and involving other clinicians as required, this specialist strand entails working with the client, employers, educators, family and friends in relevant settings and/or online to guide and facilitate engagement in education, training, and employment.
- Vestibular rehabilitation: This strand of our provision has been spearheaded by our Lead PT and now involves all other members of the therapy team, taking an interdisciplinary approach which addresses the interlinked physiological, psychological, cognitive and communication problems which may be part of the presentation for each individual client.
- Online resources: We have developed and run a suite of interactive online groups, all free of charge to anyone who has sustained an ABI, addressing a range of topics including general brain injury education and a range of specific issues including fatigue, cognition, emotions, vestibular problems, and communication difficulties. We are currently reviewing the format in which we make these resources available in order to extend their reach and accessibility, not only to people with brain injury but also to family members, support workers, and others in their network or with a general interest.
We continue to expand the range of expertise we are able to offer, by enabling our staff to develop and become proficient in new techniques and technologies. At present, for example, our physiotherapy and OT staff are being supported to undertake training in Functional Electrical Stimulation, and our neuropsychologists in specialist trauma-focused techniques. We are also working with the IT experts behind the brain injury app, Neumind, to trial its potential efficacy in enhancing our rehabilitation programmes.
Role Description
In general 47.5% – 65% of your time will be spent undertaking billed clinical work, once your caseload has built up over the first few months in post. There are likely to be occasional peaks and troughs in your clinical load, and you will be expected to take on any appropriate referrals enabling you to achieve this level of billed clinical activity as directed by the Clinical Director and COO. We will always endeavour to ensure that your cases are as geographically conveniently located as possible, but in some instances we may expect you to travel longer distances (though within your normal working hours, or with overtime payments if appropriate; see below).
As the Lead Therapist overseeing our network of associate neurophysiotherapists you will be responsible for doing all that is feasible – with central/admin support – to ensure continuity of provision to our clients.
The remainder of your time will be spent on leadership and management responsibilities, including supervision of associates, attendance at a range of meetings, representing/promoting Cognivate at various events, and providing CPD within and outside the team.
You will report directly to Dr Rose, who is your clinical line manager, and to Lydia Storr-Meddings as our business manager.
Clinical work
Your clinical responsibilities will include:
1. Undertaking physiotherapy assessments and interventions with neurologically impaired clients in their own homes, workplaces, or other community settings, collaborating and communicating with other team members and external professionals/agencies as appropriate to each case.
2. Devising and implementing high quality rehabilitation programmes, with interventions oriented towards enhancement of independence, participation and psychosocial functioning.
3. Providing interventions, advice and guidance concerning (e.g.) mobility, stamina, balance, and vestibular issues in the context of vocational rehabilitation programmes which are often interdisciplinary and involve collaboration with the client’s employers, colleagues, and teachers.
4. Collaborating with other members of each client’s interdisciplinary team to undertake initial assessments and write integrated reports concerning their needs, progress, and future treatment/management options [NB: These are usually provided in a medicolegal context but they are NOT expert witness reports].
5. Training/supervising rehab assistants and support workers, and engaging relevant members of the client’s network (e.g. family members, employers) as appropriate with physiotherapy interventions and recommendations.
6. Evaluating the impact of physiotherapy and IDT interventions through the systematic use of appropriate standardised and qualitative assessments.
7. Attending meetings, both those of the full Cognivate team and client-focused reviews/planning sessions which may involve other external agencies. The majority are virtual, but in-person Service Development Meetings involving our whole core team take place once every 6-8 weeks. You must be available to attend these and other clinical meetings on Wednesday afternoons.
8. In all respects, maintaining client confidentiality and complying with GDPR requirements as set out and kept under review by the Executive Team.
Administrative and service-related duties
9. Acting as key worker for some clients. This entails co-ordinating other Cognivate therapists who are working with the client, for example through chairing clinical meetings and discussions; overseeing the generation of integrated reports; and liaising with case managers/other external agencies as needed. Logistical aspects of this are supported by our Ops team.
10. Writing clear and concise reports in a timely manner, within a focused timeframe if required for medicolegal or other purposes, using co-designed team templates and electronic systems.
11. Keeping confidential, up-to-date and accurate written records in accordance with professional standards as set out by the HCPC and the Chartered Society of Physiotherapy [CSP]; and documenting time spent on a range of activities as required for both clinical and business purposes.
12. Generating and providing online content as required. This may include, for example, preparation of static materials (e.g. text, images, video providing accessible information) and pre-recorded educational videos.
13. Contributing to structured service evaluation and development, including through the systematic collection of outcome data and participation in training/CPD activities.
Leadership and management
14. Strategic development of the Physiotherapy department through (i) growing and managing our network of associate physiotherapists and (ii) by identifying, evaluating, and if appropriate developing and implementing expansions to our physiotherapy provision – either geographically or in terms of new services/client groups. This will be collaborative with the Executive Team and other Lead Therapists, and in due course could involve recruitment of new core physiotherapists.
15. Recruiting and overseeing associate neurophysiotherapists, and organising CPD activities and events for them and other Cognivate staff.
16. Maintaining Cognivate team members’ awareness of contemporary developments in neuro- physiotherapy which may be valuable to the quality of our provision, making recommendations concerning the development of internal expertise and potential investments.
17. Taking a lead role in shaping and implementing our clinical protocols, working with other members of the IDT to do so, and ensuring that associate physiotherapists embed these effectively in order to ensure excellence and consistency in Cognivate’s practices.
18. Giving presentations and/or contributing to exploratory discussions with potential referrers, commissioners, and professional groups including clinical practitioners, case managers, lawyers, third sector support organisations such as Headway, and employers.
19. Identifying, ordering and maintaining appropriate PT resources and equipment, ensuring adherence to professional standards of infection control and safety.
Person specification
The successful candidate will have a number of professional prerequisites including the following:
- Degree level or equivalent Physiotherapy Qualification recognised by the Chartered Society of Physiotherapy
- HCPC registration/licence to practise
- At least two years experience of working with adults who have sustained brain injury, with skills in designing and implementing tailored interventions which focus on enhancing mobility, coordination, muscle strength and activity levels to improve independence, social participation, and quality of life for people with cognitive, behavioural, emotional, and physical problems;
- At least two years experience of providing rehabilitation in community settings;
- At least three years experience of working as a member of a multidisciplinary team.
It will also be an advantage to have:
- Experience of line managing, supervising, and training more junior/less experienced physiotherapists;
- Experience of leading or making substantial contributions to the development and/or leadership of specific services;
- Training and postgraduate experience in vestibular rehabilitation
- Experience of working in a medicolegal context, either directly (e.g. having been instructed by case managers/lawyers) or indirectly (e.g. involvement in cases where clinical decisions and actions have involved discussion with legal teams and/or case managers). However all team members are supported with acquiring and developing their understanding of these issues.
Your training and experience must have equipped you with a strong understanding of how to apply contemporary models of PT practice in relation to neurorehabilitation; of confidentiality, risk management, quality assurance, and clinical governance issues; and of the Care Act 2014, continuing healthcare practice, and safeguarding processes.

- strong oral and written communication skills, enabling you to convey information in a clear, engaging, and accessible way to other team members, to clients and their families, and to other professionals working in clinical, social service, and legal sectors;
- a creative problem-solving orientation, including an openness to experimenting with new technologies to support and enhance both client and team functioning;
- an analytic approach to assessments and interventions, drawing on evidence-based practice which you actively maintain through participation in various forms of continuing professional development activities;
- a keen interest in systematically evaluating and refining team practice, both to ensure that our interventions are well-informed and to contribute to national developments in professional practice;
- the sensitivity and diplomatic skills to negotiate and compromise where appropriate – with clients and people in their family/social/support networks, with team members, and with external agencies;
- the confidence to articulate to colleagues and professionals within and outside the team the rationale for your recommendations pertaining to client rehabilitation and support needs, and to review your recommendations in light of additional information;
- understanding of, and the demonstrable ability to respond sensitively to and/or seek advice on, issues of diversity and intersectionality relating to gender, cultural background, age, disability, sexuality, and other protected characteristics.
Salary and Benefits
We are delighted to offer a competitive salary with a flexible benefits package, totalling between £55k and £65K [pro rata] depending on experience and seniority.
In addition:
- you may choose to spread your hours flexibly across the month or to work fixed days;
- you will be able to undertake overtime on billed work, should you wish to do so, at a high hourly rate;
- you will be eligible for a discretionary annual bonus;
- you will have life insurance cover should anything happen to you whilst employed by Cognivate
- you will have 35 days annual leave including bank holidays (all pro rata)
- you will have an annual allocation of time and financial support for external CPD activities
- you will have external supervision from a highly experienced neurophysiotherapist, nominated by yourself
